NOTE: Your loan application cannot be processed
until you have applied to the school.

form must sent back to the Heavy Equipment Operators School. The

Loan
amount requested'' You may add up to 60% of the cost of the program
for which you are applying to cover food and housing during your
training. Example: The tuition cost for the 6 week Heavy Equipment
Operators School is $5,495.00. Add 60% of that cost ($3,297.00)
to your "Loan amount requested,'' and the total in the box
should read $8,792.00.
NOTE:
Housing costs are outlined on the attached list.
WARNING:
Once your loan application has been approved you cannot re-apply
for a separate loan for living expenses.
Repayment term requested'' This length of time can be 2, 5 or 10
years. State
your preference.
"Interest
only payments?'' Thirty days after signing the final documents you
will begin receiving invoices. You can defer paying the principal
(And make interest only payments) for 1 month if you attend the
6 week course, and for 2 months if you attend the 9 week (6 weeks
plus 3 week CDL) course. If you check yes'' in this box put 1 or
2 months in the next box as appropriate.
We
are pleased to announce SLM Financial's new DEFERRED PAYMENT OPTION.
This option is for borrowers needing a monthly payment with the
most flexibility possible. As students work towards achieving their
career goals, they may elect to pay only $10.00 per month for up
to 12 months, regardless of class term. This $10.00 per month payment
will be applied to accrued interest during the deferment period.
Full interest and principle will begin at the end of the deferment
period.
SLM
Financial normally requires every loan applicant to have a co-borrower.
You
can speed up the loan process by including a co-borrower on your
original application. The co-borrower may be a friend or a relative.
After
you have completed, dated and signed this form, fax to: Heavy Equipment
Operators School at (541) 349-5078, or return it in the prepaid
envelope provided in the free brochure.
We
will review your application and submit it to SLM Financial for
processing. We will notify you when we hear from SLM Financial.
